Band 5 Community Nurse

Location: Chalfont’s and surrounding areas (SL9 9DR)

Contract Type: Temporary

Duration: Approx. 12 weeks

Start Date: ASAP

Working Hours: Full-time (Monday–Friday, 08:00–16:00) – minimum 4 days per week considered.

Role Overview

We are currently seeking an experienced and motivated Band 5 Community Nurse to join a community healthcare team covering Chalfont’s and surrounding areas. This role involves delivering high-quality nursing care to patients within their own homes and community settings, supporting hospital discharge and preventing unnecessary admissions.

Key Responsibilities

• Provide comprehensive nursing care to patients in community settings
• Perform wound care and dressing changes in line with clinical guidelines
• Manage and treat pressure ulcers
• Undertake catheter care (male and female), including insertion and maintenance
• Carry out venepuncture as required
• Monitor patient progress and escalate concerns appropriately
• Maintain accurate and up-to-date patient records
• Work collaboratively with multidisciplinary teams to ensure continuity of care.

Essential Requirements

• Registered Nurse (RGN) with valid NMC registration
• Previous experience in a community nursing setting
• Competency in:
  • Wound care and dressings
  • Pressure ulcer management
  • Catheterisation (male & female)
  • Venepuncture
• Full UK driving licence
• Access to a car for work purposes
• Strong communication and organisational skills

Desirable

• Experience using electronic patient record systems
• Ability to work independently in community settings.
