Location: Sirona Care and Health – Bristol / North Somerset / South Gloucestershire

Hours: Full Time

Shift Pattern: Flexible long days (Mon–Sun) – block bookings available

Pay Rate: £21 – £30 per hour

We are currently recruiting a Band 5 District Community Nurse (RGN) to join Sirona Care and Health. This is a great opportunity to work within a well-established community service, delivering high-quality care to patients in their own homes.

What You’ll Do

• Conduct comprehensive patient assessments
• Develop and implement care plans
• Deliver nursing care including medication administration and wound care
• Support patients with long-term conditions
• Promote independence and self-care
• Work collaboratively within the MDT
• Manage and prioritise a community caseload
• Maintain accurate clinical records

What You Need

• Valid NMC registration (RGN)
• Community or acute nursing experience
• Strong clinical and assessment skills
• Ability to work independently
• Excellent communication skills
• Full UK driving licence (preferred)
